She was originally seen in your department 1 week ago with the worst headache of her life. It was diagnosed as a migraine and she was discharged after anti-emetic and analgesia.

She subsequently collapsed 2 days later and died. Post-mortem has revealed it was a sub-arachnoid haemorrhage.
